Drain-and-fill versus a full flush, realistic service intervals, what lifetime fluid really means, and an honest answer to whether a transmission flush can hurt your car.
Most automatic transmissions do best with a fluid service somewhere between 30,000 and 60,000 miles, and some manufacturers extend that to 100,000 miles under normal use. Your owner's manual sets the interval for your specific vehicle, and it usually lists a shorter one for severe service. If you tow, sit in traffic, or drive a lot of short winter trips, assume you are on the shorter schedule.
These are two different services, and the words get used loosely.
A drain-and-fill empties the pan, usually with a filter change if the transmission has a serviceable filter. It typically replaces roughly a third to half of the total fluid, because the rest stays in the torque converter, cooler, and passages. It is gentle, inexpensive, and repeating it at intervals gets you most of the way to fresh fluid.
A flush, more accurately a fluid exchange, connects a machine to the cooler lines and uses the transmission's own pump to push new fluid in as old fluid comes out. Done properly it exchanges nearly all of it. Done with high pressure or reverse flow, it can dislodge debris. The exchange is a sound procedure when the equipment and fluid are right.
Two things matter more than which method you pick. The first is using the exact fluid your transmission specifies. Automatic and CVT fluids are not interchangeable, and a universal fluid with an additive packet is not the same thing. The second is the filter: if your transmission has a replaceable one, a machine exchange alone does not change it.

If your vehicle has a maintenance minder that watches operating temperature and load, that system is generally smarter than a flat mileage number. Follow it.
Several manufacturers describe their transmission fluid as filled for life. That claim reflects the design life they had in mind, and it tends to line up with a warranty period rather than with how long you intend to keep the car. It is not a claim that the fluid never degrades.
Transmission fluid does two jobs at once. It is a hydraulic fluid that applies clutches, and a friction fluid that controls how those clutches grab. Heat degrades both properties, and clutch material accumulates in it. Dark, tired fluid at 120,000 miles is not defying chemistry because a brochure said lifetime. If you plan to keep a vehicle past its warranty, servicing the fluid is cheap insurance.
This deserves a straight answer, because the internet gives you two bad ones.
A fluid exchange does not cause a healthy transmission to fail. Fresh fluid of the correct specification is better in every measurable way than worn fluid. There is no mechanism by which clean fluid damages clutches that were fine the day before.
What is true is narrower. A transmission neglected for 150,000 miles and already slipping is holding on partly because worn clutches and swollen seals compensate for each other. Fresh fluid has different friction properties than the degraded fluid it has adapted to, and a full exchange can stir up debris. Such a transmission may fail shortly after the service. It was failing before the service. The service revealed it, and sometimes hastened it by a few weeks.
So the myth is not baseless, and it is also not a reason to skip maintenance. The practical rule: service the fluid on schedule and it is nothing but good for the transmission. If you have a high-mileage transmission with unknown service history and existing symptoms, tell the shop that, and let them inspect before deciding. A conservative drain-and-fill is often the better call there.
Severe service is not exotic. It covers towing a trailer or a boat, carrying heavy loads, frequent stop-and-go driving, and repeated short trips.
Minnesota winters count. Cold thickens transmission fluid, and until it warms the pump is moving fluid that flows poorly, which means higher pressures and slower clutch apply. Short January trips never bring the fluid to operating temperature at all, and in July that same transmission may be towing in ninety degree heat. That spread is harder on fluid than a steady highway commute in a mild climate. If your driving is mostly short west-metro hops, plan on the severe-service interval.
A technician looks at three things: mileage against the manufacturer's schedule, the condition of the fluid, and how the transmission behaves.
Fluid condition is judged by color and smell. Healthy fluid is red or pink and reasonably clear. Aged fluid turns brown. Dark brown or black with a burnt smell means it has been overheated. A wipe on a clean white rag shows suspended clutch material. On a pan service the pan tells a story too: a light gray film on the magnet is normal wear, chunks or heavy sludge is not.
Right away if the fluid is burnt-smelling, if you see a leak, or if the transmission is slipping, shuddering, or shifting hard. Do not simply order a flush and hope. Have it diagnosed first.
Soon if you are well past the interval, if the fluid is dark, or if you tow and have never had it serviced.
It can wait if you are within the schedule, the fluid is clean, and the car shifts normally.
