What a rough idle means, the common causes from vacuum leaks to worn spark plugs and dirty sensors, and the one symptom that means you should stop driving.
A rough idle means the engine is running unevenly when you are stopped, and it comes down to one of three things: the engine is not getting the right amount of air, not getting the right amount of fuel, or not burning that mixture completely. The usual causes are a vacuum leak, a dirty throttle body, worn spark plugs or a failing ignition coil, clogged fuel injectors, or a dirty mass airflow sensor. On older vehicles, a sticking idle air control valve is a common culprit.
A healthy warm engine idles at a steady speed, usually somewhere around 600 to 900 RPM depending on the vehicle, and holds it. You feel a low, even hum through the seat. The tachometer needle sits still. Turning on the air conditioning or shifting into drive causes a brief dip and then a recovery.
A rough idle is the opposite. The RPM wanders up and down on its own, the whole car shakes or shudders, and you can hear the exhaust note stumble and catch. Some drivers describe the car running rough, some describe it as a misfire, and some only notice it at a stoplight. Cold mornings often make it worse, which is why a lot of Minnesota drivers first notice a rough idle in November.
Your engine measures the air it takes in so it can add the right amount of fuel. A vacuum leak lets extra air sneak in downstream of that measurement, so the mixture runs lean and the idle gets unstable. Leaks come from cracked rubber hoses, a failed intake manifold gasket, a split PCV hose, or a brittle plastic elbow.
Vacuum leaks are one of the most common rough idle causes, and they show up more in cold climates because rubber and plastic get brittle. The tell is an idle that is worse at low RPM and smooths out once you are driving, sometimes with a faint hiss under the hood.
Carbon builds up on the throttle plate and the bore around it. At idle, the throttle is nearly closed and even a thin layer of that deposit changes how much air gets through. The result is a low, lumpy idle, sometimes with stalling when you come to a stop. On drive-by-wire vehicles, the throttle body can also develop an electrical fault that sets a code and puts the car in reduced power mode.
Spark plugs wear. The gap opens up over tens of thousands of miles, and eventually the spark gets weak enough that one cylinder misfires under the low airflow conditions at idle. A failing ignition coil does the same thing more abruptly, usually on one cylinder, and often gets worse when the engine is cold or damp.
An idle misfire feels like a rhythmic shudder rather than a general roughness. If the engine smooths out at higher RPM but shakes at a stoplight, ignition is a strong suspect. This is also where a check engine light most often shows up.
Injectors spray a fine mist into the intake or directly into the cylinder. Deposits distort that spray pattern, so one cylinder gets a different amount of fuel than the rest. At highway speed you may never notice. At idle, where the fuel amounts are tiny, that difference shows up as roughness. Direct injection engines are especially prone to deposits.
Vehicles built before drive-by-wire throttles use an idle air control valve to meter air around a closed throttle plate. When it gums up with carbon or fails electrically, the idle hunts up and down, sits too low, or drops off and stalls at every stop. If you have an older car with rough idling and stalling, this is one of the first things to check.
The MAF sensor tells the computer how much air is entering the engine. Its sensing element is a fine wire or film that oil from an over-oiled aftermarket air filter, or ordinary dust, can coat. A dirty MAF under-reports airflow, the computer trims fuel accordingly, and the idle goes rough and hesitant. It often comes with poor throttle response off the line.
This one fools people. Engine mounts isolate normal engine vibration from the body. When a mount collapses or its rubber tears, that ordinary vibration goes straight into the cabin and the car shakes at idle, especially in drive with your foot on the brake. The engine itself is running fine.
The difference is in the tachometer. With a real rough idle, the RPM is unsteady and the exhaust note stumbles. With a bad mount, the RPM is rock steady and only the shaking is bad, and it usually changes noticeably between park and drive.
Not every rough idle is an emergency, but one version is.
